If equipment needs to be returned to our service department you will be required to Fax us a filled out copy of our RMA Number/Repair Order form. A copy of this form is located in the back of the manual. You can also obtain a copy from this web site by clicking the button below.

This form must be fully filled out, especially :

 THE TYPE OF PRODUCT PUMPED 

 DECONTAMINATION PROCEEDURES USED 

 SYSTEM DECONTAMINATION DATE 

Once this form is filled out Fax a copy to our service department. We will review the information and once we approve it we will issue an RMA NUMBER and fax a copy back to you. When the equipment is shipped the RMA Number MUST be marked in large letters on the outside of the shipping container.
